When the call is created, this value is the first call note that appears on the call.

What are Attached Vitals?

  • Attached vitals are the vitals that medical has created on the current call and is attached to the call.

What are Attached Narratives?

  • Attached narrative are the narrative reports that medical has created on the current call and is attached to the call.

What are Attached Forms?

  • Attached forms are the forms/reports that medical has created on the current call and is attached to the call.


Patient Tab

Value
Description

First Name

The first name of the patient.

Middle Name

The middle name of the patient.

Last Name

The last name of the patient.

Social Security Number

The social security number of the patient.

Date of Birth

The date of birth of the patient.

Weight

The weight of the patient.

Height

The height of the patient.

Gender

The gender of the patient.

Race / Ethnicity

The race of the patient.

What is Civilian Search?

  • You will utilize this feature if the patient you are treating has not been contacted by medical personnel in the past or if you are unable to retrieve their records from the patient import.

What is Patient Import

  • You will utilize this feature if the patient you are treating has received contact in the past by medical personnel. If you are unsure if they have a medical past, try entering in their name or social security number. If no results populate and they are registered within your system as a Civilian, utilize the Civilian Search.

Address

The current residing address of the patient.

City

The city where the patient lives.

County

The county where the patient lives.

Postal

The postal where the patient lives.

State

The state where the patient lives.

Phone Number

The phone number of the patient.

  • You can view if the patient has had a previous history related to conditions they have had. Medical personnel update this information after every interaction with a patient.

  • You can view if the patient has had previous allergies reported to medical personnel. You can also update if they advise of any new allergies related to the patient.

  • You can view if the patient is taking any current medication or has had any past medication. Update if necessary.


Vitals Tab

Vitals tab is a section where medical personnel record and monitor a patientโ€™s vital signs. This tab is crucial for tracking the patientโ€™s condition during transport and for providing accurate data to hospital staff upon arrival.

After you detach from a call, the vitals that you have generated will be AUTOMATICALLY DELETED. Utilize a patient care report to save your vital information.

Narrative

Narrative tab is where EMS personnel or first responders write a detailed, chronological account of the call, including their observations, actions taken, and the patientโ€™s condition throughout the encounter. Itโ€™s essentially the story of the call, written in paragraph form.

After you detach from a call, your narrative(s) will AUTOMATICALLY DELETED. Utilize a patient care report to save your narrative information.

Forms

The forms tab is a section where EMS personnel can fill out required documentation using digital versions of standardized forms. These forms are often mandated by local EMS agencies, state regulations, or federal guidelines and are essential for legal, billing, and medical records.

These forms will never be automatically deleted after detaching from a call. You will have to manually delete them from the call itself.

What is a Patient Care Report?

A Patient Care Report (PCR) is a detailed, written or electronic document that records all care, observations, assessments, and treatments provided to a patient during a medical or emergency call. It serves as a legal, medical, and administrative record of the entire patient encounter.

This form is exportable into a PDF!

Tips and Tricks

  • You can transfer all the information from the Incident, Patient, Vitals, and Narrative tabs easily into this report.

  • The data inside of this report is from the other tabs listed above. If you are unsure about a piece of information, refer to the specific tab.

  • These forms will be on a patient's history in the event of future contact.

What is a Refusal Care Report?

A Refusal of Care Report is a legal and medical document completed when a patient refuses treatment or transport after EMS or medical personnel have been called to assist. It records the patientโ€™s decision, the care offered, the risks explained, and the patient's capacity to make an informed refusal.

This form is exportable into a PDF!

Tips and Tricks

  • You can import your patient and vitals easily into this report.

  • All the values are descriptive so you should not have any trouble with the report.

Fire

What is a Fire Investigation Report?

A Fire Investigation Report is an official document that details the origin, cause, and circumstances of a fire, based on a thorough investigation conducted by fire personnel or investigators. It includes factual findings, evidence collected, witness statements, and the conclusion about how and where the fire started.

This form is exportable into a PDF!

Exporting Reports

All of our reports are exportable into PDF format. Follow our guide below to ensure these correctly format.

  1. Click Export on the report you are viewing

    1. This will NOT be available to those who are utilizing the tablet. If you would like to export a form, switch to the browser version of ImperialCAD

  2. Click More Settings on the default browser print screen

  3. Deselect the following option:

    1. Headers and Footers

  4. Select the following option:

    1. Background graphics

  5. Done! Your settings should look like this. Click Save as PDF and you will be able to save it onto your file explorer.
